Wexford PHO Moves to SPH Analytics for Improved Population Health Management

Technology helps better manage quality and cost across multiple payers for 500-provider Northern Michigan network

SPH Analytics, a leader in healthcare analytics and population health management, today announced that Wexford Physician Hospital Organization (PHO) has successfully deployed SPH’s Population Care all-payer population health management solution to achieve its quality and cost goals within the framework of value-based care. Use of Population Care is benefitting more than 77,000 patients covered by value-based contracts across the 500-provider Physician Hospital Organization in Northern Michigan by streamlining care processes, monitoring quality, and helping providers identify and close gaps in care for improved patient outcomes.
