SPH Analytics Partners with COSEHC on the Transforming Clinical Practice Initiative

The Consortium for Southeastern Hypertension Control® was recently selected to receive one of the Transforming Clinical Practice Initiative Practice Transformation Network award from the Department of Health and Human Services.

SPH Analytics, a leader in action analytics, is one of four innovative healthcare organizations that comprise the Consortium for Southeastern Hypertension Control Practice Transformation Network (PTN). In addition to SPH Analytics (SPHA), the PTN includes the Consortium for Southeastern Hypertension Control (COSEHC™), Group Practice Forum, and the Northwest Area Health Education Center (NWAHEC).

The COSEHC PTN was developed to transform care in an eight-state region spanning the southeastern portion of the United States. The COSEHC PTN will implement a practice transformation network using a pre-existing platform that can be customized and used by the Centers for Medicare and Medicaid Services (CMS) to execute the goals of the Transforming Clinical Practice Initiative. The PTN will also strongly emphasize the inclusion of small and rural practices.

The COSEHC PTN was one of 29 national and regional health care networks selected for the Transforming Clinical Practice Initiative (TCPI) award. The TCPI is one of the largest federal investments designed to support doctors and other clinicians in all 50 states through collaborative and peer-based learning networks. It is part of a collection of initiatives advanced by the Affordable Care Act to improve the quality of patient care and promote wise use of healthcare spending.

The TCPI award selection criteria focused on programs that provide clinicians with the tools and support for improving the quality of care, increasing patient’s access to information, and reducing costs.

The network will provide patient-centered quality improvement strategies and tools, data analytics, practice design, care coordination techniques, and continuous medical education to help participating clinicians meet the initiative’s phases of transformation and associated milestones, clinical and operational results.
